With decades of health care industry experience, Teresa is a respected expert in care coordination and professional case management practice. She is a prolific author with over fifty journal and magazine articles to her credit in addition to her contributions to a number of white papers and books. She is a featured columnist in Professional Case Management's Heartbeat of Case Management. Treiger is currently a Registered Nurse and a Principal with Ascent Care Management, LLC in Quincy, Massachusetts.Ellen Fink-Samnick MSW, ACSW, LCSW, CCM, CRP began her career in social work and has since flourished in many transdisciplinary leadership roles across the industry's professional associations. Hirsch This is an amazing book for case management professionals and especially for case management leaders. There is an excellent review of the history of case management and a deep analysis of the many aspects of case management. There are abundant references and case scenarios throughout each chapter.For those starting in case management, this book may be a bit too in depth but as one's experience increases with increasing responsibilities and leadership roles, this book must be read.

For professionals, by professionals - a great resource By Connie Sunderhaus Health care today requires collaboration among the variety of those in practice...from the viewpoint of a case manager; care coordinator; educator; staff nurse; leadership; clinicians; patient advocate; family practice.....and the list goes on. To fully understand the How and Why of coordination, this book is a MUST. The authors have provided an historical framework; principles of case management; standards of care and placed all into a useable tool and guide for current and future health care delivery. Anyone involved at some level of patient care coordination needs to get their hands on this book.

The authors have identified the challenges in transitioning a bedside nurse or social worker into the practice of case management. It offers practical solutions with awesome resources. This book will assist any organization or individual toward a solid outcome based Case Management program or practice.
